Ascension Health Pathways Institute is a 501(c)(3) dedicated to improving the health of our community by creating entryways into health careers, elevating the skills of the health workforce via competency based, culturally relatable instruction and empowering residents on healthy living.
Vision
To elevate community health worker impact and integration into health systems as pivotal partners in healthcare delivery through a relatable, culturally integrated, holistic and progressive method.
The AHPI CHW training program offers 140+ hours of comprehensive, cultural appropriate instruction. We include CHW Core competencies in our curriculum and health related topics such as nutrition, chronic disease management, mental health and social determinants of health and wellness.
Participants who complete the course earn a CHW certificate from AHPI as well as 12 units of community college credits from Chabot College. Additionally, participants who take three additional courses from Chabot College can earn a 2nd CHW certificate from the college. All apprentices are placed with an employer in a CHW role for up to 2000 hours.
