Ascension Health Pathways Institute Community Health Worker (CHW) Apprenticeship Program is an on-the-job training pathway that helps participants build essential CHW skills, access career opportunities in community health, and support underserved communities through improved access, self-empowerment, and chronic disease management. As a registered apprenticeship with the Department of Apprenticeship Standards, the program provides comprehensive training and education grounded in the CHW core competencies outlined by the National Council of CHW Core Consensus (C3) Standards, ensuring participants are fully prepared for successful careers in community health.

Eligibility
We encourage individuals of all ages to apply, with priority given to applicants between 18 and 24. A high school diploma or GED is required, along with a strong interest in building a meaningful CHW career after completing the apprenticeship.
