Community Health Workers

A Community Health Worker (CHW) is a frontline public health worker who is a trusted member of the community they serve. CHWs help bridge gaps between people and health systems by providing culturally appropriate outreach, health education, and connections to resources.

Two Pathways to a Career in Community Health

The CHW Apprenticeship combines hands-on training with real-world experience, ideal for those new to the field. The CHW Incumbent Training is tailored for current healthcare workers looking to enhance their skills and advance in their careers. Both programs provide the tools and support needed to make a meaningful impact in community health and advance your career!

Program Curriculum

The Community Health Worker (CHW) Workforce Development Program empowers community members to tackle health disparities through community education. Our curriculum emphasizes empowerment, leadership development and knowledge valuing the lived experiences of each participant. Our programs support community members in achieving and maintaining their CHW certification under California’s Healthcare Access and Information Department as well as the National Council on CHW Core Consensus Standards. 

Patient-Led Care

Including motivational interviewing, chronic disease management, nutrition and exercise, sexual health, substance use disorders and more.

Social Determinants of Health

Including an introduction to social determinants in our community, vaccine education, healthcare system navigation, patient advocacy and insurance coverage. 

Clinical-Skills & Documentation

Including how to take patient vitals, proper documentation and much more!
