The Ascension Health Pathways Institute Community Health Worker (CHW) Apprenticeship Program is an on-the-job training pathway that helps participants build essential CHW skills, access career opportunities in community health, and support underserved communities through improved access, self-empowerment, and chronic disease management. As a registered apprenticeship with the Department of Apprenticeship Standards, the program provides comprehensive training and education grounded in the CHW core competencies outlined by the National Council of CHW Core Consensus (C3) Standards, ensuring participants are fully prepared for successful careers in community health.

Through our partnership with Chabot College in Hayward, all apprentices are automatically enrolled and receive college credit as part of their training. Apprentices also have the opportunity to take additional no-cost courses at Chabot to earn a Community Health Worker certificate, with up to 18 months to complete the required coursework. All apprentices are required to apply to Chabot College in order to receive community college credits.
This CHW apprenticeship is an 18-month commitment that includes in-class training period followed by an externship leading into full-time employment.

APPLICATION TIMELINE:
• Applications Open: May 15th
• Applications Close: June 12th
• 1st Round Interviews (virtual): June 18th - June 24th
• 2nd Round Interviews (in-person):  June 25th - June 30th
• Admission Notifications: July 8th (Pending background check and references)
All Background Checks and References must be verified by: July 31st
 
PROGRAM DETAILS:
Program Dates: August 18th – November 13th, 2026
Location(s): Jack London Square office in Oakland, CA and Chabot College in Hayward, CA 
Scheduled In-Class Hours: 10 hours a week, classes Tuesday and Thursday – during the day
Homework Hours: 4-6 hours a week 
Community Outreach Hours: Additional outreach and experience hours are required throughout the program to provide hands-on community engagement and practical field experience.
Compensation: Stipend of up to $1,250 a month for up to 3 months of in class instruction and training.

MINIMUM QUALIFICATIONS: 
• Must be 16 years-old or older
• High school diploma or GED
• Must be legally authorized to work in the United States for the entire duration of the apprenticeship    
• Commitment to working a full-time job as a Community Health Worker at the end of training
• Excellent relationship building skills
• Excellent verbal and written communication skills
• Ability to work with a diverse group of individuals (including age, gender, race/ethnicity, etc.)
• Ability to take direction and work independently on assigned projects

REQUIRED HEALTH DOCUMENTATION:

If selected, participants must provide proof of the following prior to program start as most of our employers require. 

• COVID-19 Vaccination
Completed vaccine series per current public health and partner site requirements.

• Tuberculosis (TB) Screening
Negative TB test (blood or skin test) completed within required timeframe.

• MMR (Measles, Mumps, Rubella)
Proof of vaccination or immunity (titers accepted).

• Hepatitis B
Completed vaccine series or documented immunity (waivers may be reviewed based on placement site).

• Tdap (Tetanus, Diphtheria, Pertussis)
Up-to-date adult booster (typically within the last 10 years).

ADDITIONAL BACKGROUND ON CHWs:
Community Health Workers play an essential role in supporting patients to manage their health by helping them understand their health plan, schedule medical appointments, identify resources, and advocate for themselves. CHWs help bridge the gap between clinicians and patients by guiding them through the healthcare system, offering emotional support, and serving as a consistent point of connection. CHWs are trained in chronic disease management, motivational coaching, patient advocacy, and navigating healthcare systems.
